Opioid Conversion Table

The Perioperative Pain Management Pathway’s default standard for opioid tabs is oral morphine, 5mg, however, certain surgery pages do list oxycodone, 5mg, as the recommended discharge opioid. To convert our listed drugs/dosages to a different opioid drug, please use the table below (note that “MED” stands for “morphine equivalent dosing”). For conversions involving oxycodone, please note that the lowest possible dose of oxycodone is 50% greater than morphine.

When appropriate, we encourage prescribers to consider short-acting opioids at the lowest effective dose, with the lowest potency, for the shortest duration.
